A powerful, clot-dissolving drug known as tPA was administered, while the patient already had an anti-coagulant called Lovenox in his system. The mixing of the two drugs caused massive bleeding in the patient's brain, ultimately leading to his death.

Our client underwent an exercise stress-test, which was stopped because she developed chest pain, shortness of breath and left arm pain.
